About Evidence of Insurability

The Evidence of Insurability (EOI) requirement varies from policy to policy. Usually, Evidence of Insurability (also called "proof of good health") is required if:

  • You decline coverage during your initial eligibility period and then want coverage at a later date; or
  • You apply for Group Life Insurance in excess of the Guaranteed Issue amount.

For some plans, all increases require EOI. However, your employer will advise you if you need to provide EOI. If so, you will be asked to complete an EOI application either on paper or online at www.sunlife-usa.com/planmembers. Coverage will not go into effect until EOI is approved.
